Semen Analysis Explained: What Your Results Actually Mean
The semen analysis is one of the simplest and most informative fertility tests available — yet it's often delayed for months. Here's what the results actually mean and when to worry.
A semen analysis measures count, motility, and morphology. One abnormal result doesn't mean infertility — values fluctuate. Lifestyle changes can improve parameters within 3 months (one full sperm production cycle).
Why This Test Matters
Male factor is involved in 40–50% of infertility cases. A semen analysis takes about 30 minutes, costs $100–$300, and can either identify the problem or rule out half the diagnostic puzzle. Despite this, couples often spend months testing only the female partner before anyone checks the sperm.
The test should be one of the first steps, not the last resort.
How to Prepare
Abstain from ejaculation for 2–5 days before the test. Too short (under 2 days) may show lower concentration. Too long (over 7 days) may show reduced motility and increased DNA fragmentation. The sample is typically collected at the clinic by masturbation into a sterile cup. Some clinics allow home collection if the sample reaches the lab within 30–60 minutes at body temperature.

A Note on Morphology
This is the most misunderstood parameter. Under strict (Kruger) criteria, even fertile men typically have only 4–14% normally shaped sperm. A morphology result of 2–3% is concerning but not hopeless. Below 1% may warrant ICSI. Don't panic over morphology alone — it's evaluated alongside count and motility.
Advanced Testing: DNA Fragmentation
Standard semen analysis doesn't assess DNA quality inside the sperm. DNA fragmentation testing measures the percentage of sperm with damaged DNA. High fragmentation (>30%) is associated with lower conception rates, higher miscarriage rates, and reduced IVF success — even when standard parameters look normal.
Consider DNA fragmentation testing if you have unexplained infertility, recurrent miscarriage, failed IVF/IUI cycles despite normal standard parameters, or are over 40.
Lifestyle Factors That Improve Sperm (Within 3 Months)
Sperm take approximately 72–74 days to fully develop. Lifestyle changes today show results in about 3 months:
Heat avoidance: Skip hot tubs, saunas, and tight underwear. Don't rest laptops directly on your lap. Testicles need to be 2–4°F cooler than body temperature for optimal production.
Exercise moderately: Regular moderate exercise improves hormonal balance and sperm quality. Extreme endurance training or anabolic steroid use has the opposite effect.
Supplements with evidence: CoQ10 (200–400mg), zinc (30mg), selenium (55mcg), L-carnitine (1–3g), and vitamin C/E as antioxidants have Level 2–3 evidence for improving parameters.
Reduce alcohol and eliminate cannabis: Heavy drinking and regular cannabis use are both associated with reduced sperm count and motility.
Check medications: SSRIs, testosterone supplementation (paradoxically shuts down sperm production), finasteride, and some blood pressure medications can affect fertility. Talk to your doctor before stopping any medication.
Exogenous testosterone (TRT, steroids, testosterone gel/injections) suppresses sperm production, sometimes to zero. If your partner is on testosterone therapy and you're trying to conceive, he needs to stop and switch to alternatives like clomiphene under medical supervision. Recovery can take 6–12 months.
